Papel del gastroenterólogo en el manejo integral de las personas que viven con obesidad. Documento de Posicionamiento de la SEPD

Resumen
La obesidad constituye la enfermedad metabólica crónica más prevalente y uno de los mayores retos sanitarios y sociales de nuestro tiempo. Su estrecha vinculación con múltiples patologías digestivas –entre ellas la enfermedad hepática metabólica asociada a disfunción (MASLD), diversos cánceres digestivos, la enfermedad inflamatoria intestinal o la enfermedad por reflujo gastroesofágico– convierte al especialista en aparato digestivo en un actor clave para su detección, estratificación y manejo integral. El documento de posicionamiento de la SEPD subraya la necesidad de superar la visión limitada al tratamiento de complicaciones y situar la consulta digestiva como un punto de entrada privilegiado para la atención de las personas que viven con obesidad. La SEPD defiende un enfoque digestivo vertebrador, que integra prevención, diagnóstico avanzado, terapias farmacológicas, endoscopia y cirugía en un continuum asistencial liderado por el gastroenterólogo. De forma diferencial, el posicionamiento incorpora la endoscopia metabólica como alternativa mínimamente invasiva y coste-efectiva, puente entre fármacos y cirugía, ámbito en el que los gastroenterólogos españoles han desarrollado experiencia y liderazgo internacional. Asimismo, promueve un abordaje libre de estigma, reivindicando un lenguaje respetuoso y equitativo como parte de un compromiso ético explícito de la sociedad. La SEPD enfatiza también la necesidad de registros de vida real centrados en resultados digestivos, impulsando la investigación colaborativa y la generación de evidencia aplicable a la práctica clínica y la política sanitaria. En su conjunto, este documento reafirma el compromiso clínico, científico y social de la SEPD en la lucha contra la obesidad, con un enfoque innovador, centrado en la persona, interdisciplinar y humanista, que busca transformar la respuesta asistencial y contribuir de manera decisiva a la mejora de la salud digestiva y global de la población.
