Jejunal plasmablastic lymphoma presenting as obscure overt gastrointestinal bleeding - diagnosis by wireless capsule endoscopy

Abstract
Overt obscure gastrointestinal bleeding is one of the main indications for wireless capsule endoscopy. The widespread of this technology led to a considerable rise in the detection and, therefore, incidence, of small-bowel neoplasms. Here we describe a case of a 43 year-old male, human immunodeficiency virus-positive, that presented to our department with intermittent melena. Upper and lower gastrointestinal endoscopy showed no lesions, so he underwent a wireless capsule endoscopy. The capsule endoscopy unveiled a circumferential, stricturing and ulcerated lesion with active bleeding. The device was retained and the patient developed occlusive symptoms thus leading to an urgent surgery. The lesion was identified in the jejunum and a segmental resection was performed. Histopathology demonstrated a plasmablastic lymphoma. The patient underwent chemotherapy and after 32 months his disease is in remission. Plasmablastic lymphoma, previously considered to be a diffuse large B-cell lymphoma variant, is nowadays recognized as a separate new entity. Its occurrence in extra-oral sites is very rare and globally account for 2.6% of human immunodeficiency virus-related non-Hodgkin lymphomas. To the authors knowledge this is the first case diagnosed by wireless capsule endoscopy.
