La fragilidad física en el contexto del trasplante hepático

Resumen
En pacientes con cirrosis, la fragilidad representa una situación de disfunción física global que resulta de la desnutrición, la pérdida de masa muscular y el deterioro funcional y es una condición prevalente en los candidatos a trasplante hepático (TH). La fragilidad física ha demostrado ser un factor predictivo de resultados adversos en el contexto del TH y la evidencia apoya su incorporación a la práctica clínica no solo como factor pronóstico que guíe la toma de decisiones clínicas sino también como herramienta para identificar a pacientes subsidiarios de una intervención física y nutricional. La gran heterogeneidad de instrumentos empleados para su medición y la ausencia de un instrumento único idóneo para la evaluación de la fragilidad en todos los escenarios clínicos relacionados con el TH (hospitalización vs. ambulatorio; pretrasplante vs. postrasplante) son las principales cuestiones que quedan por resolver.
